One Hour Consultation
CONSULTING SERVICES AGREEMENT
This Consulting Services Agreement (the “Agreement”) is entered into by and between Bloom Behavior Consulting (“Consultant”), located at 3039 Richmond Rd, Woodstock, GA 30189, Phone: 863-510-8960, and the undersigned client (“Client”).
1. Scope of Services
Consultant agrees to provide Parent Consultation Sessions (“Services”) to Client. Services may be provided virtually or in person, depending on client location. Consultant will not travel more than twenty (20) miles from the business address for in-person visits. All clients located outside of this twenty (20) mile radius will receive services exclusively via telehealth. Consultant shall provide recommendations and determine whether the child will be present during sessions. Parents must be present and involved for all sessions if the child is present.
2. Payment Terms
Consultant charges $150 per hour for individual sessions. Package options include the Next Steps Package (4 hours at $499), the Bloom Package (8 hours at $899), and Playbook Bundles (varied durations). Payment must be made in full before any consultations are scheduled. Payments are processed through the website at www.bloombehaviorconsulting.net/store, where accepted payment methods are listed. Late payments are not accepted. Consultations will only be booked upon receipt of payment.
Refund Policy: All sales are final. Refunds are not available after purchase.
3. Duration and Termination
Hourly consultation sessions (1-hour consultations) must be used within thirty (30) days of purchase. Playbook bundle packages must be used within thirty (30) days of purchase. Next Steps Packages must be used within sixty (60) days of purchase. Bloom Packages must be used within one hundred twenty (120) days of purchase. Client may terminate services at any time; however, no refunds will be issued for unused hours. Consultant reserves the right to terminate services at any time, including but not limited to cases involving unprofessional or inappropriate communication, safety concerns, refusal to collaborate, or an unhealthy working environment. No refunds will be issued for terminated services under these conditions. Consultant may make exceptions for unforeseen circumstances at its sole discretion.
4. Confidentiality and Data Protection
Consultant agrees to maintain confidentiality in accordance with HIPAA standards. All notes and data collected during sessions are stored in HIPAA-compliant systems. Consultant shall not disclose any information obtained through the course of providing services except as required by law or with written consent from the Client.
5. Media and Educational Use Agreement
Client grants Consultant permission to use non-identifiable materials for educational and content creation purposes. This may include screenshots of conversations, video clips of consulting calls, and audio excerpts. Identifiable information will be removed from all posts, with the exception of parent first names. Children’s names will never be disclosed. Consultant may use these materials for training, marketing, or educational purposes to promote Bloom Behavior Consulting services.
6. Limitation of Liability
Consultant makes no guarantees or warranties regarding the outcomes of services provided. Client understands that recommendations and strategies are based on professional judgment and available information at the time of service. Consultant shall not be liable for any damages, losses, or claims arising from the Client’s implementation or non-implementation of recommendations. In no event shall Consultant’s total liability exceed the total amount paid by the Client for services rendered.
7. Dispute Resolution
This Agreement shall be governed by and construed in accordance with the laws of the State of Georgia. In the event of a dispute, the parties agree to first attempt resolution through mediation before pursuing other remedies.
8. Entire Agreement
This Agreement constitutes the entire understanding between the parties and supersedes all prior negotiations, representations, or agreements, whether written or oral. Any amendments to this Agreement must be made in writing and signed by both parties.
Note: By purchasing a consultation or package through Bloom Behavior Consulting, you agree to all terms outlined above. Upon completion of purchase, a written agreement will be sent to you for electronic signature.
